Where to Enter Your Medicare Part B Number During Sign Up

1. Visit https://www.rejuvenan.com/medicare-part-b from your internet browser and click on the 'Sign Up Now' button.
2. Fill in your information where prompted, including the field for your Medicare Part B number.
3. If you don't have your Medicare Part B number handy, click the check box that allows you to sign up for membership and provide your Medicare Part B number at a later time.
4. If you're having trouble locating your Medicare Part B number on your Medicare card, click the question mark box, which will show you where your number is located on your Medicare card and how to enter it into the field (i.e., exactly as it appears on your card, with or without dashes, checking carefully to make sure you've inputted the correct letters and numbers).
5. Click 'Sign Up.'
